(Most computers already have it.)

RTF Format (33k) Rich Text Format. This format should open in most word processing documents. If you are proficient in word processing, you may be able to type in your information, but not as easily as using the format above.

No Frills Online Version - This link will open the first page of the document, which can be printed by using your browser's PRINT button. A link at the bottom of the page will take you to the next page, which you can also print, and so on until you've opened and printed all five pages.
